Job Description

Job Summary We are seeking a compassionate and dedicated Home Health/Hospice Aide to join our team. The ideal candidate will provide essential support to patients in their homes or inpatient settings, ensuring comfort and dignity during their end-of-life journey. The Home Health/Hospice Aide will work closely with healthcare professionals to deliver high-quality patient care, assist with daily living activities, and maintain accurate medical documentation. Duties Provide personal care services to patients, including bathing, grooming, and dressing. Assist with mobility and transfers to ensure patient safety and comfort. Support patients with meal preparation and feeding as needed. Monitor and report changes in patients' conditions to the healthcare team. Maintain a clean and safe environment for patients by performing light housekeeping tasks. Document patient care activities accurately in compliance with medical documentation standards. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Provide emotional support and companionship to patients and their families during this challenging time. Requirements Previous experience in home community care or inpatient settings is preferred. Knowledge of anatomy and basic medical terminology is beneficial. Familiarity with caregiving practices and patient care protocols is essential. Experience in a medical office or clinic environment is a plus. Ability to perform phlebotomy tasks if required by the patient's care plan. Strong communication skills and the ability to work effectively within a team. Compassionate demeanor with a commitment to providing high-quality patient care. Must be able to pass background checks as required by healthcare regulations.
