Job Description

Job Summary We are seeking compassionate and dedicated Home Care Aide professionals to provide exceptional in-home and community-based care for individuals with diverse needs. As a vital member of our caregiving team, you will support clients in maintaining their independence, ensuring their safety, and enhancing their quality of life. This role offers an opportunity to make a meaningful difference by delivering personalized care that promotes dignity, comfort, and well-being. The ideal candidate is energetic, detail-oriented, and committed to upholding the highest standards of patient-centered care. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, vital signs, and behavioral changes to inform care plans Administer medications safely following HIPAA guidelines and prescribed instructions Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Support clients with memory care needs such as Alzheimer's or dementia by implementing specialized techniques Utilize equipment such as Hoyer lifts or other assistive devices to ensure safe patient transfers and heavy lifting when necessary Maintain detailed documentation of care provided, patient observations, and adherence to care plans in compliance with healthcare regulations Perform light cleaning duties to ensure a safe and hygienic environment for clients Collaborate with healthcare professionals, social workers, and family members to deliver comprehensive support Provide emotional support and companionship to foster a positive environment for clients in assisted living or home & community care settings Experience Prior experience working in home health, assisted living, nursing homes, or group homes is highly preferred Demonstrated knowledge of patient observation, medication administration, and vital signs monitoring Experience caring for individuals with developmental disabilities, autism spectrum disorder, or Alzheimer's/dementia is advantageous Familiarity with HIPAA regulations and maintaining confidentiality in healthcare settings Hands-on experience with patient monitoring tools and mobility aids such as Hoyer lifts or resident lifts Background in long-term care or hospice environments is a plus Strong communication skills for working effectively with patients, families, and healthcare teams Join us if you're passionate about providing compassionate care that transforms lives! Pay:
$22.60 - $28.83 per hour
