Full Job Description TITLE:
Overnight Direct Support Professional (Awake)

Part Time/ Full Time Immediate Supervisor:
Program Manager/Director of Independent and Supported Living Services Supervision Exercised:
None SUMMARY:
Under the supervision of the Program Director and Manager, this position will provide awake overnight support services to individuals living in independent and supported living settings, promoting safety, independence and dignity for persons served.
ESSENTIAL FUNCTIONS
Record and appropriately utilize necessary data, progress notes, staff logs, and program required documentation Supervise and assist persons served in following their overnight and daily morning schedule including household meal preparation, maintaining sanitary and orderly conditions in their home, other household tasks and transportation as necessary Prompt and assist persons served in medication management as needed Assists participants in all aspects of personal care Assist persons served in weight bearing and non-weight bearing transfers Use crisis intervention techniques in emergency situations when persons served are in danger of injuring themselves or others Participates in in-service training and staff meetings to facilitate professional development Complies with all Momentum Work Inc. Policies and OSHA policies and procedures Other related duties, responsibilities and special projects on an as needed basis
PHYSICAL REQUIREMENTS
Ability to stand, sit for long periods of time Ability to go up and down stairs Ability to walk for long periods of time Ability to push or pull wheelchairs when necessary Ability to speak, read, hear, and understand technical written materials The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served Full awareness of environmental stimuli Ability to implement Crisis Management, Safety & Pro Act techniques (training included)

QUALIFICATIONS
High school diploma or equivalent required; Associate of Arts degree or equivalent in related field preferred Experience working with persons with developmental and/or other disabilities Demonstrated ability to maintain confidentiality regarding person served and his/her related information Excellent verbal and written communication skills Ability to work with little direct supervision Compassion and respect for persons with disabilities May need to be available during any disasters, per disaster plan Must have reliable phone Must have personal car and valid insurance Must have valid CA Driver's License (if applicable) Valid certification in CPR/1st Aid (Training available)
