In this vital role, you will provide essential support to patients in their homes, promoting independence, comfort, and well-being. Your energy and attention to detail will help create a safe, nurturing environment for those with diverse health needs, including seniors, individuals with disabilities, and patients requiring specialized care such as Alzheimer's or dementia care. This position offers an opportunity to work closely with healthcare professionals and empower clients to live their best lives at home. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient health status by checking vital signs and reporting changes promptly Administer medications accurately following prescribed instructions while maintaining compliance with HIPAA regulations Support patients with medical procedures such as catheter care, Hoyer lifts, and other assistive devices Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Provide companionship and emotional support to enhance mental well-being, especially for those with dementia or Alzheimer's disease Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients Collaborate with healthcare teams to coordinate care plans and document patient progress effectively Assist in hospice or palliative care settings by providing compassionate support aligned with patient wishes Requirements Proven experience in home health care, assisted living, nursing homes, or long-term care environments Knowledge of medical terminology and procedures relevant to patient care and medical documentation Certification in CPR and first aid is required; additional certifications such as hospice or Alzheimer's care are a plus Ability to work independently while following detailed care plans and protocols Strong communication skills to interact effectively with patients, families, and healthcare professionals Experience working with individuals with developmental disabilities or disabilities in general is highly valued Familiarity with EMR/EHR systems for accurate record keeping and documentation Physical ability to perform caregiving tasks including lifting using Hoyer lifts and assisting with mobility needs Join us in delivering exceptional home health services that prioritize dignity, respect, and personalized care.
