The Position is local to the South San Francisco area and nearby Bay Area communities. What You'll Do Provide personal care (bathing, grooming, dressing, toileting, transfers) Assist with mobility, prescribed exercises, and activities of daily living Take and record vital signs; observe and report changes to the RN Case Manager Assist with meal preparation and light housekeeping related to the care plan Document care accurately using agency systems (training provided) Follow the physician's Plan of Care and agency policies What You'll Bring (Requirements) Active California CHHA Certification (required) CNA certification (preferred) Current BLS/CPR certification Valid California driver's license, reliable transportation, and active auto insurance Current TB screening (within last 12 months) Current health assessment and required immunizations/titers (Hep B, MMR, Varicella, Tdap) Strong communication skills and a genuinely caring attitude Previous home health or caregiving experience preferred What We Offer Competitive pay: $24
