In this energetic role, you will provide compassionate, personalized care and support to residents in various settings, including group homes, assisted living facilities, and community-based programs. Your positive attitude and proactive approach will foster a safe, respectful environment where each person's dignity is prioritized. Duties Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility to promote independence and comfort Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly Support medication administration following HIPAA guidelines to ensure safety and compliance Prepare nutritious meals in accordance with care plans and dietary restrictions, ensuring proper food handling and cleanliness standards Maintain a clean and safe environment through regular cleaning tasks and sanitation procedures Document care activities accurately in compliance with HIPAA regulations and facility policies Engage residents in social activities and provide companionship to promote mental well-being and community involvement Follow established protocols for emergency response including CPR and first aid when necessary Collaborate with team members on individualized care plans tailored to each resident's unique needs, including dementia, Alzheimer's, autism spectrum disorder, or other disabilities Skills Proven experience in group home settings, assisted living, nursing homes, or home & community care environments Knowledge of patient observation techniques and vital signs monitoring for effective patient care management Strong understanding of medication administration procedures and HIPAA privacy standards Ability to perform light housekeeping tasks such as cleaning and meal preparation efficiently Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease, autism spectrum disorder or memory care needs Certifications such as CPR, first aid, or nursing assistant credentials are mandatory; willingness to obtain necessary certifications are required Excellent communication skills for working collaboratively within multidisciplinary teams including social workers and healthcare providers Join us if you're passionate about providing compassionate support that transforms lives!
