  • Part-time Job Overview Join our compassionate team as a Home Health Aide & Caregiver, where you will play a vital role in enhancing the quality of life for individuals in their homes and community settings.
This energetic position involves providing personalized care, assisting with daily activities, and supporting clients with a range of health and wellness needs. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and document patient observations, vital signs, and medical conditions using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications and perform specialized tasks such as catheter care, Hoyer lift transfers, and hospice or dementia care Support patients with memory care needs, Alzheimer's care, and working with individuals with developmental disabilities or disabilities Maintain accurate medical documentation while ensuring compliance with HIPAA regulations for patient privacy and confidentiality Prepare nutritious meals and assist with meal planning to promote health and well-being Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment in accordance with home & community care standards Use medical terminology appropriately when recording notes or communicating with healthcare professionals Respond promptly to patient needs during live-in employment or scheduled visits, ensuring compassionate and respectful care at all times Experience Prior experience in home health care, assisted living facilities, nursing homes or long-term care environments is highly preferred Demonstrated proficiency in patient observation, vital signs measurement, and medical documentation practices Familiarity with EMR/EHR systems for accurate record keeping and reporting Experience working with individuals who have dementia, Alzheimer's disease, or other cognitive impairments is advantageous Knowledge of medication administration procedures and safety protocols including CPR certification is required Hands-on experience with specialized equipment such as Hoyer lifts or mobility aids is a plus Ability to work independently or as part of a team in diverse settings while maintaining professionalism and compassion Join us to provide essential home health services that empower clients to live safely and comfortably.
