Job Summary A Physicians Home Care (APHC) is a locally owned business in Colorado Springs that was started by a family of a Special Needs child who is now an adult. We founded in 2017 in hopes to be able to provide the best possible care to those who need in- home assistance. Our motto is where everyone is treated as family and our main goals are to give back and help others. We have had the pleasure of helping over 900 families and are licensed by the state of Colorado to provide In Home Support Services (IHSS) as well as Accredited by the state and ACHC to provide CNA, Skilled Nursing, and Therapy Services. We are looking for a reliable home health client support staff/back-up caregiver who recognize, like us, that the world of health care is constantly changing and are motivated to do what is right. Responsibilities:
Travel to consumers home to assess clients and write up care plans that will be followed by various clinicians Communicate with the appropriate office personnel on client needs and assist in coordinating care with family members Conduct Supervisory Visits In-home competency Communicate with Case Managers to be able to best meet the client's and family's needs Protect and respect the confidentiality of all client records and relationships and keep all client personal health information according to policy Complete requisite paperwork and documentation on time and accurately Following agency policies and procedures Follows state regulations and office policies and procedures Perform other functions as directed by the Supervisor/ Agency Manager, (Items may include, but not limited to filing, phone coverage or special projects) Trains new hire caregivers Primary back-up caregiver
Responsibilities:
- Assist as assigned with activities of daily living such as bathing, grooming, toileting and elimination, and adequate nutritional intake.
- Assist as assigned with ambulation, transfers and/or range of motion exercises.
- Ability to lift up to 100 pounds
- Light Housekeeping
- Read and record client temperature, pulse and respiration as assigned.
- Recognize and document changes in client condition and report to supervisor.
- Provide Medical Oversight to ensure our clients remain safe and free from injury.

Education:
High school/GED or equivalent desirable Basic First Aid and CPR are desirable, but not required
Experience:
