Job Description
Job Summary We are seeking a compassionate and dedicated Day Program Support Specialist to assist individuals with developmental disabilities and other needs in a community-based setting. The ideal candidate will provide personalized support, promote independence, and ensure safety while fostering a positive environment for participants. This role involves working closely with clients to assist with daily activities, health monitoring, and community engagement, contributing to their overall well-being and quality of life. Duties Assist participants with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and document vital signs and health status in accordance with care plans Administer medications and assist with medication management following HIPAA guidelines Support participants during meal preparation, cooking, and nutritional planning Provide personal care such as hygiene assistance and light cleaning of living areas Utilize specialized equipment like Hoyer lifts for safe transfers and mobility assistance Observe and report changes in behavior or health status through patient observation techniques Implement individualized care plans tailored to each participant's needs, including dementia, Alzheimer's, autism, or other disabilities Engage clients in social activities and community outings to promote socialization and independence Maintain a safe environment by adhering to safety protocols including CPR and first aid procedures Ensure compliance with all healthcare regulations, including HIPAA standards for confidentiality Experience Prior experience working in assisted living, home & community care, or long-term care settings preferred Experience working with individuals with developmental disabilities or special needs such as autism or dementia is highly desirable Knowledge of patient monitoring, medication administration, and vital signs measurement is advantageous Previous roles involving patient care, nursing home experience, hospice or home health care are beneficial Familiarity with care plans, resident support strategies, and social work principles is a plus Certifications such as CPR and First Aid are preferred but not required; training will be provided if necessary Ability to perform heavy lifting when necessary for client transfers or mobility assistance This position offers an opportunity to make a meaningful difference in the lives of individuals requiring dedicated support.
