In this role, you will support residents in their daily activities, promote independence, and ensure a safe and nurturing environment. The ideal candidate will have experience in home and community care, assisted living, or long-term care settings, with a strong focus on patient-centered support. This position offers an opportunity to make a meaningful difference in people's lives through personalized care and support. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance Support residents with medication administration and monitor vital signs as needed Provide patient observation to identify changes in health or behavior and report findings appropriately Prepare nutritious meals according to dietary guidelines and assist with feeding when necessary May need to utilize lifting equipment such as Hoyer lifts to safely transfer residents with mobility challenges Maintain cleanliness of the living environment through cleaning duties Follow Individual plans to ensure individualized support for each resident Document care activities accurately while adhering to HIPAA regulations in Therap Engage residents in social activities and promote community involvement through rec activities. Respond promptly to emergencies with appropriate first aid or CPR if required Collaborate with healthcare professionals, family members, and team members to deliver comprehensive care Requirements Prior experience in group homes, assisted living, nursing homes, or home & community care preferred Knowledge of patient care practices including Alzheimer's care, dementia care, memory care, autism support, hospice care, and senior care is highly desirable Certification in CPR and First Aid is preferred; willingness to obtain certification if not already certified Ability to work with individuals who have disabilities or developmental challenges using specialized techniques Strong interpersonal skills with patience and empathy for residents' needs Ability to work independently and as part of a team in a fast-paced environment Physical ability for heavy lifting and assisting with mobility tasks Knowledge of HIPAA regulations related to patient privacy and confidentiality Experience in medication administration and vital signs monitoring is advantageous Flexibility to work various shifts including days, evenings, or weekends as needed for coverage Ability to work collaboratively with other staff in a professional manner. Must be reliable and punctual. 1st shifts 7a-3p or 8a-4p, 2nd shifts 2p-8p, 3p-10p, 3rd shifts 5p-9a or 9p-9a This role is vital in providing compassionate support that enhances the quality of life for residents.
