Job Description

Join our compassionate team as a Personal Care Assistant and make a meaningful difference in the lives of individuals requiring support with daily activities and personal care. This energetic role offers the opportunity to provide vital assistance in home and community settings, ensuring safety, dignity, and comfort for those you serve. Your dedication will help promote independence and enhance quality of life for residents with diverse needs, including seniors, individuals with disabilities, or those experiencing memory care challenges. Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote independence and dignity. Monitor patient health by checking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals. Support medication administration following care plans and HIPAA regulations to ensure safety and compliance. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces. Use specialized equipment such as Hoyer lifts or transfer belts to safely assist with mobility and prevent injury during transfers. Document care activities accurately in compliance with facility policies and coordinate with healthcare teams to update care plans regularly. Proven experience in home health, assisted living, nursing homes, or group home settings is highly desirable. Certifications in First Aid, CPR, and HIPAA compliance are required; additional training in Alzheimer's care, dementia care, or hospice care is a plus. Ability to work effectively with individuals with developmental disabilities, autism spectrum disorder, or memory impairments such as dementia. Strong observational skills for patient monitoring and recognizing changes in condition or behavior. Physical ability to perform heavy lifting tasks safely and assist with patient transfers using appropriate equipment like Hoyer lifts. Excellent communication skills for working collaboratively with clients, families, and healthcare professionals.
