Job Description

Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals needing support at home. In this vital role, you will provide personalized assistance to clients with diverse needs, including elderly individuals, those with disabilities, or patients recovering from illness. Your energetic approach and commitment to patient well-being will foster independence, comfort, and dignity for every person you serve. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Monitor and observe patients' conditions regularly, noting vital signs and changes in health status to ensure prompt response to any concerns. Assist with medication reminders following documentation and HIPAA guidelines to maintain patient safety and confidentiality. Support clients with meal preparation and nutritional needs, ensuring dietary restrictions are followed while promoting healthy eating habits. Maintain detailed medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, documenting care provided and patient progress thoroughly. Provide companionship and emotional support, especially for individuals with dementia or Alzheimer's disease, creating a safe and engaging environment. Perform light housekeeping tasks such as cleaning, laundry, and organizing to promote a clean and safe living space for clients. Requirements Proven experience in home health care or assisted living environments; long-term care experience is highly valued. Knowledge of catheter care, hospice care, dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities or disabilities. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as HIPAA training are preferred. Ability to work independently while following detailed care plans and medical instructions. Strong communication skills to collaborate effectively with clients' families and healthcare teams. Experience operating EMR/EHR systems for accurate medical documentation. Physical ability to assist with patient mobility using equipment like Hoyer lifts and perform tasks requiring standing, bending, or lifting safely. Pay:
$24.45 - $27.15 per hour People with a criminal record are encouraged to apply
