Job Description

Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide exceptional support and care to individuals in various settings, including assisted living, home health, and community care environments. In this vital role, you will assist clients with daily activities, monitor their health status, and ensure their safety and comfort. Your energetic approach and attention to detail will help enhance the quality of life for those you serve. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or assisting with transfers. Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately in care plans. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Administer medications following prescribed instructions while maintaining compliance with HIPAA regulations to protect patient privacy. Support clients with memory care needs, including dementia and Alzheimer's care, ensuring a safe environment and engaging activities. Perform light housekeeping duties such as cleaning client living areas to maintain a hygienic environment. Collaborate with healthcare professionals to develop and follow personalized care plans that meet each individual's unique needs. Ensure safety during patient transfers using proper techniques and equipment like Hoyer lifts or resident's . Provide emotional support and companionship to promote social interaction and mental well-being. Experience Prior experience working in assisted living facilities, nursing homes, or home & community care settings is highly preferred. Experience working with individuals with developmental disabilities, autism, dementia, or memory care needs is advantageous. Certification in first aid and CPR is required; additional training in hospice or long-term care is a plus. Knowledge of patient observation techniques, medication administration, vital signs monitoring, and HIPAA compliance is essential. Demonstrated ability to handle heavy lifting safely and operate patient transfer equipment effectively. Familiarity with creating and implementing care plans tailored to individual requirements is desirable. Strong interpersonal skills with a compassionate approach to caregiving are essential for success in this role.
