Job Description

Job Summary We are seeking a compassionate and dedicated Caregiver & Home Health Aide to provide exceptional support and personalized care to individuals in their homes or assisted living environments. In this vital role, you will assist clients with daily activities, ensure their safety, and promote independence while maintaining a warm, respectful, and professional demeanor. Your efforts will help improve quality of life for those you serve, fostering a safe and nurturing environment that promotes dignity and well-being. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications as prescribed, ensuring proper documentation in compliance with HIPAA regulations. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory care needs through tailored activities and compassionate engagement. Support hospice or palliative care patients by offering emotional comfort and managing specific medical needs such as catheter care or hospice protocols. Prepare nutritious meals aligned with dietary restrictions and assist with feeding when required; perform light housekeeping tasks including cleaning and laundry to maintain a safe living environment. Use medical terminology accurately when documenting patient records and communicating with healthcare teams. Operate medical equipment safely, including Hoyer lifts, blood pressure monitors, and other assistive devices to promote patient safety. Work collaboratively within home & community care settings to deliver comprehensive support tailored to each individual's needs while respecting privacy laws like HIPAA. Qualifications Proven experience in home health care, assisted living, nursing homes, or long-term care environments. Strong knowledge of patient observation techniques, medication administration, and medical documentation procedures. Familiarity with EMR/EHR systems for accurate recordkeeping; experience working with individuals with disabilities or developmental disabilities is preferred. Certifications such as CPR (Cardiopulmonary Resuscitation), first aid training, and any state-specific caregiving licenses are highly desirable. Ability to perform physical tasks including lifting patients using Hoyer lifts or other assistive devices; comfort with providing personal care activities. Excellent communication skills to effectively interact with clients, families, and healthcare professionals while maintaining confidentiality under HIPAA guidelines.
