In this vital role, you will provide essential support to patients in their homes, promoting independence, safety, and well-being. Your energetic approach and attention to detail will help create a comfortable and nurturing environment for those in need of home health services. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. All roles are paid positions, emphasizing the value we place on your contribution. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and catheter care when necessary Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; document medical records accurately using Electronic Health Record (EHR) systems Observe and report on patient condition changes, ensuring timely communication with healthcare providers Support medication administration following proper protocols to ensure safety and compliance with HIPAA regulations Provide specialized care for individuals with dementia or Alzheimer's disease through memory care techniques and tailored activities Assist with meal preparation and nutritional needs while maintaining cleanliness and sanitation standards in the home environment Perform light housekeeping tasks such as laundry, cleaning, and maintaining a safe living space to promote overall health and comfort Requirements & Qualifications Active Home Health Aide (HHA) certification in good standing, if required by state regulations High school diploma or GED equivalent preferred Current résumé required One to two professional references required Current negative TB test result or willingness to obtain one before employment Ability to pass a criminal background check Valid government-issued photo identification Authorization to work in the United States Current CPR and First Aid certification preferred Valid driver's license and reliable transportation preferred Ability to safely assist clients with ambulation, transfers, mobility, bathing, grooming, dressing, toileting, and other activities of daily living (ADLs) Ability to lift, transfer, and reposition clients in accordance with agency policies and safety procedures Strong interpersonal, communication, and documentation skills Commitment to maintaining client confidentiality and HIPAA compliance Professional, dependable, compassionate, and patient-centered approach to care Ability to work independently in clients' homes while maintaining communication with agency staff Willingness to complete agency-required orientation, training, and continuing education Experience providing home care, personal care assistance, companionship care, dementia care, Alzheimer's care, hospice support, or care for individuals with disabilities preferred but not required Ability to accurately document services provided and report changes in client condition to the appropriate supervisor Embark on a rewarding career where your caregiving expertise directly enhances the quality of life for those you serve.
