Full Job Description Overview:
The Home Health Aide provides direct consumer care as assigned by Open Systems Health in the consumer home and community. To provide quality, safe and effective home care services while adhering to the independent living philosophy of the consumer. Responsibilities:
Ensure quality and safe delivery of home care services that align with OSH, federal and state policies procedures as well as the Occupational Safety and Health Administration and CDC recommendations. Participate in development and implementation of consumer plan of care in accordance to OSH policy and procedures. Establish and maintain constructive and cooperative interpersonal relationships with consumers. Assist consumers with personal care services as per their care plan, which may include bathing, grooming, assistance with toileting, changing urinary drainage bags, transfer/ambulation/exercise assistance, assistance with self-administration of medications, reading and recording temperature, pulse and respiration, observing/recording/reporting the consumer's physical condition/behavior/appearance, meal preparation and assistance, infection control, light housekeeping, simple dressing changes that do not need the skills of a licensed nurse, assisting with activities that are directly supportive of skilled therapy services and routine care of prosthetic and orthotic devices. Utilize Universal Precautions while providing care. Report any unusual incidents or changes in the consumer's behavior to the case manager. Communicate all relevant consumer information with OSH in a confidential and timely manner. Accurate documentation of services provided according to the consumer's plan of care. Participate in annual trainings. Organizing, planning, and prioritizing work. Comply with Federal, State and Local Laws. Comply with all OSH policy and procedure requirements.
Qualifications:
Required Qualifications:
High School diploma or GED equivalency, preferred. HHA certification that meets State and OSH standards. Evidence of Tuberculosis screening that meets CDC guidelines. Evidence of competency as per OSH and State requirements. Must have a reliable form of transportation. Passed OSH competency exam with a score of 80% or greater.

Knowledge of appropriate and safe techniques in all aspects of personal care. Ability to manage multiple tasks on a daily basis and manage time efficiently.
