Ability to work independently in a home-based care setting What You'll Do Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and mobility Support with household tasks such as meal prep, light cleaning, laundry, and grocery shopping Monitor and report changes in patient condition (vital signs, behavior, skin, mobility, etc.) Observe medication routines and report concerns to the care team Document care provided accurately using electronic systems Work collaboratively with a multidisciplinary healthcare team to support patient care plans Qualifications High school diploma or equivalent Minimum 1 year of experience working with elderly or frail populations Strong communication skills and professionalism in patient homes Ability to work independently and manage a daily route Comfortable using basic technology (email, documentation systems) Must pass background check and meet health screening requirements About the Role You'll be providing essential, hands-on care that helps patients maintain independence and safely remain in their homes.
