In this vital role, you will provide compassionate, personalized support that promotes independence, safety, and well-being. Your energy and commitment will help create a nurturing environment where clients feel valued, respected, and empowered to thrive. This paid position offers an opportunity to develop your caregiving skills while making a positive impact every day. Duties Assist clients with Day Habilitation and Community Participation Services. Monitor patient health by taking vital signs, observing for changes in condition, and reporting findings accurately in compliance with HIPAA regulations. Support and implement individualized care plans tailored to each client's needs. Assist with meal planning to promote healthy eating habits. Maintain a clean and safe environment by performing light housekeeping Provide companionship and emotional support to foster social interaction and reduce feelings of isolation. Document care activities thoroughly, ensuring compliance with healthcare standards such as HIPAA and agency policies. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings is highly desirable. Certification in First Aid and CPR is required; Ability to work with individuals with developmental disabilities, autism, dementia, or memory care needs with patience and understanding. Strong observational skills for patient monitoring and recognizing changes in health status. Excellent communication skills to collaborate effectively with team members, clients, families, and healthcare professionals.
