Job Description

Empath Personal Care, a part of Empath Health, is seeking a dedicated and compassionate Home Health Aide Trainees to serve patients in Hillsborough or Pinellas County within their homes. In this role, the Home Health Aide Trainee provides personal care, activities of daily living, and support services to meet various care needs of the clients while advocating independence and maintaining functional ability. The position is responsible for appropriately documenting patient care for assigned clients. Provide compassionate personal care and assistance with daily living activities to patients in their homes. Support patients' comfort, dignity, and independence through individualized care. Communicate effectively with patients, families, and healthcare team members to ensure coordinated service. Observe and report changes in patient condition to the appropriate clinical staff. Travel within the assigned service area to deliver care as scheduled. Provide compassionate personal care and assistance with daily living activities to patients in their homes. Support patients' comfort, dignity, and independence through individualized care. Communicate effectively with patients, families, and healthcare team members to ensure coordinated service. An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
