We are seeking a compassionate and dedicated Home Health Aide to join our team and provide exceptional in-home care to clients. As a vital member of our healthcare family, you will assist individuals with their daily living activities, monitor their health status, and ensure their comfort and safety. This role offers an opportunity to make a meaningful difference in the lives of those needing support with dignity, respect, and personalized attention. You will work closely with clients, families, and healthcare professionals to deliver comprehensive home health services that promote independence and well-being. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems. Support medication administration following prescribed schedules and protocols while maintaining strict adherence to HIPAA regulations for patient confidentiality. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques. Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively. Document medical records accurately in EMR systems and communicate updates to healthcare teams promptly. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Assist with hospice care needs by providing emotional support and comfort measures aligned with patient wishes. Support clients with disabilities or those in assisted living or long-term care settings by delivering personalized care plans. Maintain compliance with all safety standards including CPR certification and proper handling of medical supplies. Requirements Proven experience working as a Home Health Aide or in a similar caregiving role within home health, nursing homes, assisted living facilities, or long-term care environments. Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation purposes. Certification in CPR is required; additional certifications such as hospice care or dementia training are highly desirable. Ability to perform patient observation accurately and respond swiftly to changing health conditions. Experience working with individuals with developmental disabilities or chronic conditions like Alzheimer's disease is preferred. Strong communication skills to collaborate effectively with healthcare teams and families while maintaining HIPAA compliance. Physical stamina to assist patients with mobility support using Hoyer lifts or other assistive devices. Flexibility to work live-in shifts or varied schedules based on client needs.
