As an HHA, you will play a vital role in supporting individuals with their daily activities, ensuring their safety, comfort, and well-being. This position offers an opportunity to make a meaningful difference in the lives of those requiring assistance with aging, disabilities, or recovery. The ideal candidate is energetic, detail-oriented, and committed to delivering personalized patient care in a respectful and professional manner. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; accurately document medical records and updates in Electronic Health Record (EHR) systems Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral management techniques Assist with medical procedures such as catheter care, wound dressing changes, and hospice support as directed by healthcare professionals Prepare nutritious meals tailored to dietary needs and assist with feeding when required; perform light housekeeping tasks including cleaning and laundry to maintain a safe environment Utilize medical terminology knowledge to communicate effectively with healthcare teams and document patient progress clearly and thoroughly Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred Demonstrated experience caring for individuals with developmental disabilities or chronic conditions such as dementia or Alzheimer's disease Familiarity with EMR (Electronic Medical Records) or EHR systems for documentation purposes Long-term care experience involving patient observation, medication administration, and vital signs monitoring Hands-on experience using mobility aids like Hoyer lifts or other assistive devices in patient transfers Knowledge of hospice care principles and working compassionately with patients nearing end-of-life stages Proven ability to work independently while maintaining excellent communication skills within a caregiving team Join us in delivering compassionate home health services that empower individuals to live safely and comfortably at home.
