license, or Home Health Aide Certificate (75 hours minimum training) from a Florida Dept. of Education approved program Level 2 background screening with ACHA fingerprints Physical within (6) months that states you are

FREE OF COMMUNICABLE DISEASES

(cannot be signed by a Chiropractor) CPR (American Heart Association, American Red Cross, or American Safety & Health Institute)

HIV/AIDS

certificate (minimum 1 hour) Alzheimer's/Dementia Certificate (2 HOUR) approved by
