Job Description

Overview Join our compassionate team as a Home Health Aide and make a meaningful difference in the lives of individuals requiring personalized care at home. In this vital role, you will provide essential support to patients with diverse needs, ensuring their safety, comfort, and independence. Your dedication will help promote health, well-being, and dignity for those under your care, fostering a nurturing environment that empowers clients to thrive in their daily lives. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, accurately documenting vital signs and health status in electronic health record (EHR) or electronic medical record (EMR) systems Administer medications and perform basic medical procedures such as catheter care and Hoyer lift transfers under supervision or as trained Support patients with memory care needs, including Alzheimer's and dementia care, ensuring a safe and calming environment Provide companionship and emotional support while engaging patients in meaningful activities tailored to their interests and abilities Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation Follow HIPAA regulations strictly to protect patient confidentiality and ensure compliance with healthcare standards Skills Proven experience in home health or long-term care environments, including assisted living or nursing homes Strong knowledge of medical terminology, vital signs monitoring, medication administration, and documentation practices Skilled in working with individuals with developmental disabilities or those requiring hospice or memory care services Proficient in using EMR/EHR systems for accurate medical records management and medical documentation Ability to operate medical equipment such as Hoyer lifts, blood pressure cuffs, thermometers, and other patient care devices Excellent caregiving skills with a compassionate approach to patient care and a keen eye for patient observation Valid CPR certification and experience with hospice or dementia-specific care are highly desirable Comfortable working independently in patients' homes while adhering to safety protocols and healthcare regulations Join us to provide top-tier home health services that enhance quality of life.
