Full Job Description Position Summary :
Under the direction of the Registered Nurse or Therapist, provide personal care services. The Home Health Aide/Certified Nurse's Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort. Participates and is an active member of the patient's interdisciplinary treatment team.
Job Responsibilities:
Provide care as defined by the RN/PT/OT/ST and a Home Health Aide Care Plan. Communicates status/condition changes to the
RN/PT/OT/ST
per agency guidelines. Promotes personal safety and a safe environment for clients and coworkers Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist such as range of motion or assistance with ambulation or exercise. Assists/provides patient with ADLs to include bathing, shampooing, skin/nail care, oral hygiene, shaving and/or dressing. Performs household services essential to health care at home, including but not limited to Meal preparation, laundry or light housekeeping. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs. Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures. Respects confidentiality of information in the client clinical record and only shares this information in accordance with Agency policy and HIPAA Responsible to report weekly schedule and report if over/under productivity. Acts in a professional manner, work as scheduled and report to work on time. Follow procedures to work within the frame of the law and scope of practice. Accurately document care provided onto notes/patient's chart. Ensure that all documentation is completed in a timely manner within the agency. Represent the Agency at all times in a professional and loyal manner. Attend meetings and in-services as directed. Maintains accurate, comprehensive, and professional written documentation. Perform other duties as may be required from management.
Job Qualifications:
Successful completion of a formal certification-training program and a written skills test and competency evaluation. Be at least eighteen (18) years of age. Minimum of six (6) months' work experience in a supervised setting, preferably home health care or a health care facility. Demonstrated ability to read, write, and follow a written Plan of Care. Good verbal, written, and interpersonal skills. Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record. Hands-on BLS Card. Maintains confidentiality of all patient information. Adheres to policies and procedures specific to patient rights. Maintains a clean and safe environment; identifies and reports hazards. Assures patients, guest, clients, and residents safety. Meets all requirements for mandatory in-services. Adheres to policies/procedures specific to universal precautions when delivering patient care. Adheres to policies regarding attendance, conduct, grooming, and dress code. Exhibits excellent service to patients, visitors, physicians, and co-workers. Shows courtesy, compassion and respect for all customers. Promotes the mission, vision, and values of the organization. Complies with professional, regulatory, ethical, and legal standards.
