In this vital role, you will provide personalized assistance to promote independence, safety, and well-being for clients across diverse care settings. Your energetic approach and attention to detail will help create a nurturing environment that fosters trust and comfort for those in your care. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, recording vital signs and noting any changes in condition Support medication administration following proper protocols and documentation procedures Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately document care activities and medical information Provide specialized care such as Alzheimer's, dementia, memory care, hospice, or working with individuals with developmental disabilities Operate assistive devices like Hoyer lifts and perform catheter care to ensure safety and comfort Prepare nutritious meals, assist with feeding, and maintain cleanliness of the living environment including laundry and light cleaning tasks Requirements Proven experience in home health, assisted living, nursing homes, or long-term care environments Knowledge of medical terminology and procedures related to patient care Certifications such as CPR (Cardiopulmonary Resuscitation) are required; additional certifications like HIPAA compliance are preferred Ability to work with individuals with disabilities or cognitive impairments such as Alzheimer's disease or dementia Strong patient observation skills to detect changes in health status promptly Experience with medical documentation and familiarity with EMR/EHR systems for accurate record keeping Compassionate attitude with excellent communication skills and a commitment to providing high-quality patient care Join us in delivering exceptional home health services where your expertise directly enhances the quality of life for those you serve.
