Job Description

Overview Join our compassionate team as a Home Health Aide & Caregiver, where your dedication and skills will make a meaningful difference in the lives of individuals needing support at home or in community settings. This vital role involves providing personalized care, assisting with daily activities, and ensuring the safety and comfort of our clients. Whether working with seniors, individuals with disabilities, or those recovering from illness, you'll be an essential part of their well-being and independence. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Monitor and observe patients' conditions, including vital signs and overall health status, documenting findings accurately. Support clients with specialized care needs including Alzheimer's care, dementia care, hospice support, and working with individuals with developmental disabilities or disabilities. Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to maintain a safe and comfortable environment. Document medical records thoroughly and communicate effectively with healthcare providers regarding patient progress or concerns. Qualifications Prior experience in home health care, assisted living, nursing homes, or long-term care environments preferred Knowledge of medical terminology, medication administration, and patient care procedures Certification in CPR is required; additional certifications such as hospice care or dementia training are a plus Ability to work independently while following detailed care plans and medical documentation protocols Experience working with individuals with developmental disabilities or memory impairments such as Alzheimer's disease is highly valued Strong communication skills to effectively collaborate with clients, families, and healthcare professionals Valid driver's license and reliable transportation required.
