The ideal candidate will assist with activities of daily living, monitor patient health, and support individuals with diverse needs, including seniors, those with disabilities, and individuals requiring specialized care such as Alzheimer's or dementia. This role offers an opportunity to make a meaningful difference in patients' lives through personalized care and support. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance Monitor and observe patient conditions, reporting changes or concerns promptly Administer medication and manage treatments following healthcare provider instructions Perform medical tasks such as taking vital signs, catheter care, and wound management Support patients with memory care needs, Alzheimer's, dementia, and hospice care requirements Utilize Electronic Medical Records (EMR/EHR systems) to document patient information accurately Assist with meal preparation and feeding to ensure proper nutrition Perform light housekeeping duties such as cleaning and laundry to maintain a safe environment Operate medical equipment like Hoyer lifts safely and effectively Follow HIPAA regulations to ensure patient confidentiality and privacy at all times Collaborate with healthcare teams to coordinate comprehensive home & community care services Requirements Proven experience in home health, assisted living, nursing homes, or long-term care environments preferred Knowledge of medical terminology, medication administration, and vital signs monitoring Certifications such as CPR are required; additional certifications in hospice or dementia care are a plus Experience working with people with developmental disabilities or disabilities preferred Ability to work independently while maintaining high standards of patient care Strong communication skills and compassionate demeanor Ability to work flexible hours including live-in employment options if applicable Must pass background checks and comply with health screening requirements Valid driver's license preferred for travel between client locations This position is ideal for caring professionals committed to delivering quality home health services while fostering independence and dignity for their clients.
Pay:
$15.00 - $17.00 per hour
