In this vital role, you will provide comprehensive support to a single client in their home, promoting independence, safety, and well-being. Whether assisting with daily activities or offering specialized care, your efforts will foster a nurturing environment that empowers the client to thrive. This position offers the rewarding opportunity to build trusting relationships while delivering exceptional patient-centered care. Responsibilities Assist client with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe client health status by taking vital signs such as blood pressure, pulse, and temperature; document findings accurately Support medication administration following prescribed protocols while maintaining compliance with HIPAA regulations to ensure confidentiality Provide specialized care for the client with cognitive decline, including memory care techniques and behavioral management strategies Perform light cleaning and laundry to maintain a safe and hygienic environment Document medical records thoroughly, including patient observations, medication administration logs, and care notes Collaborate with healthcare teams to coordinate home care plans and ensure seamless communication across providers Support the client by implementing a personalized care plans to promote independence and dignity Experience Prior experience in assisted living facilities, nursing homes, or home health environments is highly preferred Demonstrated ability to work effectively with seniors and individuals with disabilities or chronic conditions such as dementia or Alzheimer's disease Hands-on experience with medical terminology, vital signs monitoring, medication administration, and basic nursing procedures Knowledge of HIPAA compliance standards to protect patient privacy and confidentiality Familiarity with medical documentation practices Experience working with individuals who have developmental disabilities or require hospice or memory care services is a plus Certification in CPR is required; additional certifications such as hospice care training or catheter care are advantageous Ability to perform physical tasks including lifting patients safely using Hoyer lifts or other assistive devices Join us in delivering compassionate home care that transforms lives!
