JOB DESCRIPTION SUMMARY
The Home Health Aide is a member of the home care team who works under the supervision of a care manager and performs various personal care services as necessary to meet the clients' needs. The Home Health Aide is responsible for observing clients, reporting these observations and documenting observations and care performed. The Home Health Aide will be assigned in a manner that promotes quality, continuity, and safety of a clients' care.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Responsibilities of the Home Health Aide include, but are not limited to the following: Provide client care as directed by the Care Manager.
Provide personal care:
b athing, grooming, including oral care Assist the client with dressing and undressing Assist the client with toileting activities Assist in feeding the client, if necessary Feed clients who are unable to feed themselves Take and record vital signs (temperature, pulse, respirations and blood pressure) Measure and record height and weight, if necessary. Support good skin care to prevent skin breakdown (i.e. turn and reposition client, actively encourage movement) Assist client with active and passive range of motions as defined by the plan of care Provide client with help walking and transferring Observe clients physical, mental and emotional conditions. Report any change to the Care manager Document observed changes Measure and document intake and output, if necessary Assist client in the self-administration of medication (medication reminder) Perform light housekeeping Perform meal preparation and clean up Change bed linens and make up the clients' bed Maintain an orderly client environment. Follow principles of infection control and Universal Precautions Identifies and participates in performance improvement activities. Provide transport to doctor appointments, outside activities and shopping Participates in client teaching according to the clients' plan of care Assist in providing a safe environment and promote quality patient care through adherence to established policies, procedures and standards. Ensure clients rights are adhered to include but are not limited to the right to privacy and dignity while care is being provided, the right to accept or refuse care, and the right to confidentiality of their information Respond rapidly and appropriately in an emergency situation Use equipment properly and in a safe manner.
QUALIFICATIONS
High school diploma Completion of a home health aide program ( 40 hour class) from a community college or vocational school, or authorized home health agency (with a certificate of completion) Comply with state and local regulations Minimum of one (1) year of documented experience required Negative TB skin test or chest x-ray Self-directing with the ability to work with little direct supervision Demonstrate oral and written communication skills Ability to express spoken and/or written ideas in English Works as a team member
JOB LIMITATIONS
The Home Health Aide will not function in any manner viewed as the practice of nursing according to practice acts. Specifically, the Home Health Aide will not administer medications, take physicians orders or perform procedures requiring the training, knowledge and skill of a Licensed Nurse.
ENVIRONMENT, PERSONAL PROTECTIVE EQUIPMENT OR PHYSICAL DEMANDS
Working Conditions:
Contact with client in their living environment. May be required to respond in an emergency situation. Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.
Personal Protective Equipment:
May include gloves, mask, eye protection, CPR shield and disposable outer covering.
Physical Requirements:
Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift up to 50 pounds on a regular basis.
NOTE:
You may also start as a Homemaker/Companion (with a lower pay rate) and complete a free Home Health Aide course paid for by employer (with a pay raise after completion).
