Job Description

Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail make a meaningful difference in the lives of individuals needing support at home. In this vital role, you will provide personalized assistance to patients, helping them maintain independence and quality of life. Your expertise in patient observation, medication administration, and assisting with activities of daily living (ADLs) will ensure each patient receives safe, respectful, and comprehensive care. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care, all within a supportive and dynamic environment. Duties Provide direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts. Monitor and document vital signs, health status, and changes in condition accurately using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems. Support patients with medication reminders and administer medications following proper protocols while adhering to HIPAA regulations for confidentiality. Assist with specialized care needs including catheter care, Alzheimer's and dementia care, hospice support, and memory care routines. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients. Observe and report patient responses to treatments or changes in condition promptly to healthcare professionals. Maintain detailed medical documentation and ensure compliance with all safety standards and healthcare regulations. Requirements Proven experience as a Home Health Aide or in a similar caregiving role within assisted living facilities, nursing homes, or home health settings. Knowledge of medical terminology, patient care procedures, and caregiving best practices for individuals with disabilities or chronic conditions. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or dementia certification are a plus. Ability to work independently while following detailed care plans; excellent observational skills for patient monitoring. Experience working with individuals with developmental disabilities or long-term care needs preferred. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Strong communication skills to collaborate effectively with healthcare teams and family members while maintaining patient confidentiality under HIPAA guidelines.
