Job Description

Job Title:
LSD1 / Companion (Life Skills Development Level 1)
Service Description:
LSD1/Companion services are designed to provide supervision, assistance, and training to support the client in developing and maintaining essential daily living skills. The goal is to promote independence, safety, and community integration while ensuring the client's needs are met in accordance with their support plan.
Primary Responsibilities:
Provide direct supervision to ensure the client's safety in the home and community Assist with the development of daily living skills, including personal routines and structure Offer companionship and social support to reduce isolation and encourage engagement Support the client in participating in community-based activities and outings Assist with maintaining a clean, safe, and organized living environment Provide reminders and guidance for daily tasks (e.g., hygiene, meals, appointments) Help the client build appropriate social and communication skills Monitor and document the client's progress toward established goals Follow all APD guidelines, service authorizations, and client support plans Report any significant changes, incidents, or concerns to the provider/supervisor promptly
Additional Duties:
Encourage independence while providing only the level of assistance necessary Support the client in making appropriate choices and decisions Maintain professional boundaries and confidentiality at all times Assist with basic meal preparation and light housekeeping (as outlined in the support plan) Provide transportation or accompany client to approved activities (if applicable and authorized)
Qualifications:
High school diploma or equivalent (required) Experience working with individuals with developmental disabilities (preferred) Ability to follow written service plans and document services accurately Strong communication and interpersonal skills Ability to remain patient, supportive, and attentive Valid driver's license (if transportation is required) Completion of required APD training and background screening
Work Environment:
Services are provided in the client's home and community settings May require evenings, weekends, or flexible scheduling based on client needs Goal of the
