Job Description

Overview We are seeking a compassionate and dedicated Home Health Aide to provide essential support to individuals in need of assistance with daily living activities. The ideal candidate will have a strong commitment to patient care, with the ability to observe and report changes in the health status of clients. This role is crucial in ensuring the comfort and well-being of patients, particularly those with conditions such as dementia. Duties Assist clients with daily living activities, including bathing, dressing, grooming, and meal preparation. Monitor and record vital signs, ensuring any changes are reported to healthcare professionals promptly. Provide companionship and emotional support to clients, fostering a positive environment. Perform light housekeeping tasks such as laundry and cleaning to maintain a safe and comfortable living space for clients. Assist clients with mobility needs, including lifting or transferring them safely when necessary. Adhere to HIPAA regulations and maintain confidentiality regarding patient information. Support clients in assisted living environments by following individualized care plans tailored to their specific needs. Engage in patient observation to identify any changes in behavior or health status that may require attention. Experience Level 2 Background Check, CPR, Drivers License, and
HIV/ADRD
training required. CNA license or Home Health Aide certificate preferred. Previous experience in caregiving or a related field is preferred but not required; training will be provided. Familiarity with dementia care techniques is a plus. Knowledge of patient observation practices and vital sign monitoring is beneficial. Ability to work effectively within various systems related to patient care and documentation. Strong interpersonal skills and the ability to build rapport with patients and their families are essential.
