Job Description
Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and proactive support will make a meaningful difference in the lives of individuals needing assistance with daily activities and health management. This role offers an energizing opportunity to provide personalized home and community care, ensuring safety, comfort, and dignity for our clients. As a vital member of the caregiving team, you will work closely with patients, families, and healthcare professionals to deliver exceptional service in a variety of settings. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and document patient health status by observing vital signs and reporting changes promptly Support medication administration following proper protocols and medical documentation standards Provide specialized care such as Alzheimer's care, dementia care, hospice support, and working with individuals with developmental disabilities or disabilities Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately record patient information and medical records Use assistive devices like Hoyer lifts safely to aid patient mobility and prevent injury Perform household tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients Requirements Proven experience in home health or long-term care environments; nursing home experience is highly valued Knowledge of medical terminology, HIPAA regulations, and medical documentation practices Certification in CPR and first aid is required; additional certifications such as catheter care or hospice care are a plus Ability to work independently and as part of a team in live-in or home-based settings Strong patient observation skills with the ability to recognize changes in health status or behavior Experience working with individuals with disabilities or memory impairments such as Alzheimer's disease or dementia Valid driver's license and reliable transportation preferred for community-based visits Join us in delivering compassionate, energetic care that empowers individuals to live comfortably at home.
