Home Health Aide (Per Diem) Independent Living Systems - 2.8 Sunrise, FL Job Details Full-time 19 hours ago Qualifications High school diploma or GED Full Job Description We are seeking a Home Health Aide to join our team at Independent Living Systems (ILS). ILS, along with its affiliated health plans known as Florida Community Care and Florida Complete Care, is committed to promoting a higher quality of life and maximizing independence for all vulnerable populations. The Home Health Aide plays an essential role in providing compassionate and personalized care to individuals who require assistance with daily living activities in their own homes. This position ensures the safety, comfort, and well-being of members by delivering high-quality support tailored to their unique health needs. The Home Health Aide collaborates closely with healthcare professionals and family of the members to implement care plans effectively and monitor member progress. By fostering a supportive and respectful environment, the Home Health Aide helps enhance the quality of life for members, promoting independence whenever possible. Ultimately, this role contributes significantly to the continuum of care by enabling members to remain in their homes safely and comfortably.
Minimum Qualifications:
High school diploma or equivalent. Completion of a state-approved Home Health Aide training program. Certification as a Home Health Aide (HHA) or equivalent credential as required by the state. Basic knowledge of healthcare techniques and safety protocols. Ability to pass background checks and meet health screening requirements.Preferred Qualifications:
Experience in assisted living or long-term care settings. CPR and First Aid certification. Familiarity with electronic health records (EHR) and computer data entry. Strong interpersonal and communication skills. Additional training in specialized care areas such as dementia or chronic disease management.Responsibilities:
Assist members with activities of daily living such as bathing, dressing, grooming, and mobility support. Monitor and record member's vital signs and health status, reporting any changes to nursing staff or supervisors promptly. Provide medication reminders and assist with prescribed treatments as directed by healthcare professionals. Maintain a clean, safe, and sanitary environment in the member's home to prevent infections and promote health. Support members with meal preparation, feeding assistance, and ensuring proper nutrition and hydration. Document care provided and update member records accurately using computer data entry systems.
