Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor and observe patients' conditions, including vital signs and overall health status, documenting changes accurately in Electronic Health Record (EHR) systems Administer medications and manage medical documentation in compliance with HIPAA regulations and care plans Support clients with meal preparation, feeding, light cleaning, laundry, and household chores to maintain a safe and comfortable environment Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions through tailored memory care techniques Assist with medical procedures such as catheter care, wound care, or hospice support as needed under supervision or according to training Collaborate with healthcare teams using EMR systems to ensure seamless communication about patient progress and updates on medical records Requirements Prior experience in home health care, assisted living, nursing homes, or long-term care environments preferred Knowledge of medical terminology and familiarity with medical documentation practices Certifications such as CPR (Cardiopulmonary Resuscitation) are required; additional certifications like medication administration or hospice care are a plus Experience working with individuals with disabilities or developmental disabilities is highly valued Ability to perform patient observation accurately and respond swiftly to changing needs Strong communication skills and a compassionate attitude toward patient care Valid driver's license and reliable transportation for community-based visits Join us today if you're passionate about providing high-quality home health services!
