In this vital role, you will provide essential support to promote independence, comfort, and well-being for patients with diverse needs, including seniors, individuals with disabilities, and those receiving hospice or memory care. Your energetic approach and attention to detail will ensure each patient receives high-quality, respectful care tailored to their unique circumstances. •Dog Friendly•
Schedule:
Thursday 8pm-8am Friday 8pm-8am Sunday 10am-6pm
Duties:
Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Support medication administration following prescribed instructions while maintaining strict adherence to HIPAA regulations for patient confidentiality. Observe and document patient conditions diligently, noting any changes or concerns related to health or behavior. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and ensure a safe environment. Assist with household tasks such as light cleaning, laundry, meal preparation, and ensuring a hygienic living space. Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to update medical documentation promptly and accurately. Collaborate with nursing staff and other healthcare professionals to coordinate comprehensive patient care plans. Support patients in community settings or long-term care environments by providing consistent, compassionate care tailored to individual needs. Engage in home care activities when applicable, offering comfort and emotional support to patients and families. Skills Strong patient observation skills with the ability to identify changes in health status quickly. Experience working in assisted living facilities, nursing homes, or home health environments. Proficiency with EMR/EHR systems for medical documentation and record keeping. Knowledge of assisting with ADLs, medication administration, catheter care, and hospice or dementia-specific needs. Familiarity with medical terminology relevant to home health caregiving. Certification in CPR is required; additional certifications such as HIPAA training are preferred. Ability to operate medical equipment like Hoyer lifts safely and effectively. Excellent communication skills for working with patients of varying abilities and collaborating with healthcare teams. Compassionate caregiving attitude with a focus on patient dignity and safety. Ability to perform light housekeeping duties including cleaning and laundry while maintaining a professional demeanor.
