Job Description

Job Summary Join our dedicated team as a Home Health Aide & Caregiver, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals requiring assistance with daily activities and health management. In this vital role, you will provide personalized support in patients' homes or community settings, ensuring safety, comfort, and dignity. Your expertise will help promote independence and enhance quality of life for those with diverse needs, including seniors, individuals with disabilities, or those recovering from illness. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor patient health through observation of vital signs and overall condition, reporting changes promptly Administer medications following prescribed schedules and document medication administration accurately in medical records or electronic health record (EHR) systems Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, or hospice care Operate medical equipment including Hoyer lifts, catheters, and other assistive devices to ensure safe patient handling Prepare nutritious meals and assist with meal planning while adhering to dietary restrictions and preferences Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment in compliance with HIPAA regulations Document all patient interactions and medical information clearly using electronic documentation systems to ensure accurate medical records Experience Prior experience providing home health care or caregiving services in assisted living facilities or nursing homes Familiarity with working with individuals with developmental disabilities or disabilities requiring long-term care Knowledge of medical terminology relevant to patient care and medication management Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or dementia-specific training are a plus Experience using EMR (Electronic Medical Records) or EHR systems for documentation purposes Demonstrated ability to observe patient conditions carefully and respond appropriately to emergencies or changes in health status Strong communication skills to collaborate effectively with healthcare teams and family members Join us in delivering compassionate home health services that empower individuals to live safely and comfortably. $15.00 - $17.00 per hour
