Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote dignity and independence Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals Support medication reminder Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately document patient care activities and medical records Prepare nutritious meals and assist with light housekeeping tasks like laundry and cleaning to ensure a safe living environment Support patients during transportation for medical appointments or community outings while maintaining safety protocols Requirements Proven experience in home health care, assisted living, nursing homes, or long-term care environments Knowledge of medical terminology Certification in CPR (Cardiopulmonary Resuscitation) is required. Ability to operate EMR/EHR systems accurately and maintain strict confidentiality in accordance with HIPAA standards Strong communication skills to collaborate effectively with healthcare teams and families Physical stamina to perform caregiving tasks that may involve lifting or assisting patients with mobility needs Valid driver's license and reliable transportation preferred for home visits Join us in delivering compassionate home health services that empower clients to live safely and comfortably.
