This position offers an exciting opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, monitoring health conditions, and supporting overall well-being. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver attentive and respectful care that promotes independence and comfort. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to ensure patient health stability. Support medication administration following prescribed instructions while maintaining accurate medical documentation in Electronic Health Record (EHR) systems. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral management. Assist with medical procedures such as catheter care, wound care, and using assistive devices like Hoyer lifts or other mobility aids. Help with meal preparation and nutritional needs while ensuring food safety and dietary restrictions are followed. Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and organizing patient spaces. Document all patient interactions accurately in compliance with HIPAA regulations to protect patient confidentiality. Support hospice or palliative care needs by providing compassionate emotional support and comfort measures. Requirements Proven experience working as a Home Health Aide or in a related senior or home care setting; experience in assisted living or nursing homes is highly valued. Knowledge of medical terminology, vital signs monitoring, medication administration, and basic medical procedures. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are preferred. Ability to work independently while following detailed care plans and instructions from healthcare professionals. Strong communication skills to effectively interact with patients with developmental disabilities or cognitive impairments like Alzheimer's disease. Familiarity with EMR/EHR systems for documentation purposes; experience with home & community care software is advantageous. Physical ability to assist with patient transfers using equipment like Hoyer lifts and perform tasks requiring mobility support.
