Job Description
The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) provides compassionate, high-quality care to patients in the comfort of their homes. Responsibilities include assisting with personal care, light housekeeping, meal preparation, companionship, and other activities of daily living to help patients remain safe, independent, and comfortable in their home environment. All care is provided under the supervision of a Registered Nurse and in accordance with the patient's plan of care, Florida AHCA regulations, Medicare Conditions of Participation, and agency policies and procedures. Key ResponsibilitiesPersonal Care Assistance Assist patients with bathing, grooming, dressing, oral care, and personal hygiene. Provide assistance with ambulation, transfers, positioning, and mobility. Assist with toileting and incontinence care. Support the use of assistive devices, including wheelchairs, walkers, canes, and commodes. Assist with prescribed exercises and range-of-motion activities as directed. Basic Health Monitoring Obtain and record vital signs, including temperature, pulse, respiration, and blood pressure, as assigned. Observe and report changes in the patient's physical, emotional, or environmental condition to the supervising nurse immediately. Monitor intake and output and perform simple urine tests, if appropriately trained and assigned. Homemaker and Companion Services Perform light housekeeping duties in patient care areas, including laundry, dishwashing, and general tidying. Prepare and serve meals according to patient preferences and dietary restrictions. Assist with grocery shopping, errands, and other approved household tasks. Provide companionship and emotional support while maintaining professional boundaries. Documentation and Compliance Complete and submit visit documentation accurately and timely. Maintain all required records in accordance with agency, state, and federal requirements. Ensure documentation supports payroll processing, patient care, and regulatory compliance. Follow HIPAA guidelines and maintain strict patient confidentiality. Communication and Team Collaboration Maintain regular communication with the clinical team and office staff regarding patient status and scheduling. Respond promptly to phone calls, messages, and requests from supervisors. Participate in staff meetings, in-services, and required training programs. Additional Responsibilities Recognize and appropriately respond to emergency situations. Adhere to infection control and safety practices. Maintain a professional appearance and conduct at all times. Keep certifications, credentials, and required screenings current. Perform other duties as assigned to support patient care and agency operations. Qualifications Current Florida Home Health Aide (HHA) certificate (minimum 40-hour training program) or active Florida CNA license. Current CPR certification. Valid driver's license, reliable transportation, and proof of automobile insurance. Ability to travel to patient homes within assigned coverage areas. Ability to follow written and verbal instructions. Strong communication and interpersonal skills. Compassionate, dependable, professional, and patient-focused. Previous home health experience preferred but not required. AHCA Requirements Meets all Florida Agency for Health Care Administration (AHCA) requirements for home health personnel. Successful completion of Level II Background Screening. Current TB screening and health records as required by state regulations. Eligible to work in the United States.
