Job Description

Job Overview Join our compassionate team as a Home Health Aide, where you will play a vital role in delivering personalized, high-quality care to individuals in their homes. This energetic and rewarding position offers the opportunity to make a meaningful difference in the lives of patients by providing essential support with daily activities, health monitoring, and emotional well-being. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to promote independence and comfort in a safe, caring environment. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to ensure patient stability Administer medications accurately following medical documentation protocols and HIPAA regulations Support patients with specialized care needs such as Alzheimer's, dementia, memory care, hospice, and developmental disabilities Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe and hygienic environment Document patient progress in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems with attention to detail and compliance Assist with catheter care, wound management, and other medical procedures under supervision or as trained Observe patient conditions closely for any changes or concerns related to health or safety and communicate effectively with healthcare teams Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred Demonstrated ability to work with individuals with disabilities or complex medical needs such as Alzheimer's or dementia care Knowledge of medical terminology and familiarity with medical documentation standards Certification in CPR is required; additional certifications such as medication administration or hospice care are advantageous Experience using EMR/EHR systems for accurate recordkeeping is preferred Long-term care experience combined with home & community care background will set candidates apart Proven ability to handle caregiving responsibilities with compassion, patience, and professionalism Join us in providing exceptional home health services that empower individuals to live safely and comfortably at home. $12.00 - $15.00 per hour
