Job Description

Overview We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Personal Care Assistants (PCAs) to join our vibrant caregiving team. In this vital role, you will provide exceptional patient-centered care to individuals in assisted living, nursing homes, home health, and community settings. Your energy and attention to detail will help improve quality of life for those with diverse needs, including seniors, individuals with disabilities, and those requiring specialized dementia or Alzheimer's care. Duties Assist residents and clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient observations by tracking vital signs, responding to changes in condition, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following proper protocols and documentation standards while ensuring patient safety Provide specialized care such as catheter care, hospice support, dementia and memory care activities, and Alzheimer's care strategies Operate assistive devices like Hoyer lifts safely to transfer patients with mobility challenges Assist with meal preparation, feeding, cleaning, laundry, and maintaining a clean and safe environment in compliance with health regulations Document medical records meticulously and maintain confidentiality in accordance with HIPAA regulations Collaborate effectively with healthcare teams to ensure comprehensive patient care plans are followed Experience Prior experience working in nursing homes, assisted living facilities, or home health environments preferred Proven ability to work with individuals with developmental disabilities or cognitive impairments such as dementia or Alzheimer's disease Familiarity with medical terminology and medical documentation practices essential for accurate record-keeping Experience operating EMR/EHR systems for patient data entry and management Knowledge of hospice care principles and long-term care procedures highly desirable CPR certification required; additional certifications such as medication administration or catheter care are a plus Demonstrated ability to perform patient observation accurately and respond promptly to changing needs Experience working with individuals with disabilities or in community-based care settings is advantageous Join us in providing compassionate, high-quality care that transforms lives!
