  • Part-time, PRN Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients.
In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and independence. Whether working with seniors, individuals with disabilities, or those recovering from illness, your efforts will promote comfort and well-being every day. This position offers an opportunity to develop rewarding relationships while delivering essential home health services in a supportive environment. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient condition through regular observation of vital signs and overall health status. Administer medications and manage medical documentation in compliance with HIPAA regulations and EMR/EHR systems. Support patients with specialized care needs such as Alzheimer's, dementia, memory care, hospice, or catheter care. Operate medical equipment like Hoyer lifts and perform tasks such as patient transfers safely and efficiently. Maintain a clean and safe environment by assisting with light housekeeping tasks including laundry and meal preparation. Document patient progress accurately in medical records and communicate updates to healthcare professionals as needed. Experience Prior experience in home health care, assisted living facilities, nursing homes, or long-term care settings is highly valued. Demonstrated ability to work effectively with individuals with developmental disabilities or complex medical conditions such as dementia or Alzheimer's. Familiarity with medical terminology, medication administration protocols, and medical documentation processes. Experience using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate record keeping. Certification in CPR and first aid is required; additional certifications like hospice care or working with patients with disabilities are a plus. Proven ability to handle sensitive situations with compassion while maintaining patient confidentiality under HIPAA guidelines. Strong communication skills to collaborate effectively with clients, families, and healthcare teams.
