Job Description

Join our compassionate team as a Personal Care Assistant and make a meaningful difference in the lives of individuals requiring support with daily activities and personal care. In this vital role, you will provide attentive, respectful, and personalized care to promote independence, safety, and well-being. Whether working in a home setting, assisted living facility, or group home, your dedication will help create a nurturing environment where clients feel valued and cared for every day. Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and dignity. Monitor patients' health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals. Support medication administration following prescribed care plans while adhering to HIPAA regulations for patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking as required. Maintain a clean and safe environment by performing light cleaning duties and ensuring proper sanitation practices are followed. Use specialized equipment such as Hoyer lifts or other assistive devices to safely transfer residents with mobility challenges. Document care activities meticulously, including patient observations, progress notes, and updates to care plans to ensure continuity of care. Proven experience working in home health, assisted living, nursing homes, or group homes with individuals who have developmental disabilities, dementia (including Alzheimer's), autism spectrum disorders, or other special needs. Certifications in First Aid and CPR are required; additional training in hospice care or memory care is a plus. Knowledge of HIPAA regulations and experience with patient monitoring, medication administration, and vital signs measurement. Ability to lift heavy objects safely and perform physically demanding tasks related to patient transfers and caregiving duties. Strong communication skills for collaborating with healthcare teams, residents' families, and other caregivers. Experience working with diverse populations including seniors, individuals with disabilities, or those requiring long-term care support.
