Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and record vital signs such as blood pressure, temperature, pulse, and respiration rates accurately using Electronic Health Record (EHR) systems Observe patient conditions closely, noting changes in health status or behavior related to dementia, Alzheimer's disease, or other cognitive impairments Support medication administration following proper protocols while maintaining strict adherence to HIPAA regulations and medical documentation standards Provide specialized care for individuals with developmental disabilities or those requiring hospice or memory care services Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams to ensure continuity of care Requirements Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments Knowledge of medical terminology and proficiency with EMR (Electronic Medical Records) and EHR systems Certification in CPR and training in catheter care, dementia care, hospice support, and medication administration preferred Ability to work independently and demonstrate excellent patient observation skills Experience working with individuals with disabilities or developmental challenges is highly valued Strong understanding of HIPAA compliance and patient privacy standards Flexibility to work live-in shifts or part-time hours as needed to meet client needs Compassionate attitude combined with excellent communication skills to build trust with patients and families Embark on a rewarding career where your dedication transforms lives!
