In this vital role, you will provide high-quality, patient-centered care across a variety of settings, including hospitals, long-term care facilities, assisted living communities, and home care environments. Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote independence and comfort. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately using EMR (Electronic Medical Records) systems. Support medical teams by preparing patients for procedures, assisting with medical documentation, and ensuring adherence to HIPAA regulations for patient privacy. Provide specialized care for individuals with dementia or Alzheimer's disease, utilizing memory care techniques to ensure safety and dignity. Administer medications and perform basic medical tasks such as catheter care, wound dressing changes, and phlebotomy under supervision when qualified. Operate patient lifts such as Hoyer lifts safely to prevent injury during transfers. Maintain a clean and safe environment by following aseptic techniques and infection control protocols in accordance with hospital or facility standards. Skills Proven experience working with diverse populations including seniors, pediatrics, individuals with developmental disabilities, or behavioral health needs. Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for accurate medical documentation. Strong knowledge of physiology, anatomy, medical terminology, and vital signs monitoring. Experience in various healthcare settings such as ICU (Intensive Care Unit), emergency medicine, post-acute care, hospice care, or outpatient clinics. Skilled in patient service activities including medication administration and basic life support certification. Ability to work effectively within high-pressure environments like Level I Trauma Centers or Level II Trauma Centers while maintaining attention to detail. Compassionate caregiving skills combined with excellent communication abilities to work with patients of all ages—including children—and collaborate seamlessly with multidisciplinary teams.
