Responsibilities:
Communicate well with the clients, families and other support professionals. Handle requests and inquiries from clients, families and other professionals. Help clients carry out their responsibilities in the community. Work with care providers to find solutions to clients' needs. Ensure effective communication with other service providers. Maintain client and upcoming schedule. Maintain a clean and safe environment. Review and evaluate client's daily activities.,.
