Job Description

Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide essential support and care to individuals in various settings, including assisted living facilities, home health environments, and community-based programs. The ideal candidate will possess a strong commitment to enhancing the quality of life for clients by delivering personalized care with professionalism, empathy, and attention to detail. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring assistance with daily activities and medical needs. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patients' health status by observing vital signs, medication adherence, and overall well-being, ensuring compliance with care plans. Support patients with medication administration and recordkeeping in accordance with HIPAA regulations and facility policies. Prepare nutritious meals tailored to individual dietary requirements while maintaining cleanliness and safety standards. Utilize equipment such as Hoyer lifts and other assistive devices to safely transfer residents or clients with mobility challenges. Maintain a clean, safe, and organized environment by performing light cleaning duties in client living spaces or care facilities. Collaborate with healthcare professionals and social workers to develop and implement personalized care plans that meet each individual's needs. Provide companionship and emotional support to promote mental well-being, especially for clients with dementia, Alzheimer's, or developmental disabilities. Document patient observations accurately and report any changes or concerns promptly to supervising staff. Comply with all safety protocols including CPR, first aid procedures, and infection control standards during caregiving activities. Experience Prior experience working in assisted living facilities, nursing homes, home health care, or group homes is highly desirable. Demonstrated knowledge of patient observation techniques, vital signs monitoring, and medication administration. Experience caring for individuals with dementia, Alzheimer's disease, autism spectrum disorder, or other developmental disabilities. Familiarity with care procedures such as feeding assistance, light housekeeping related to caregiving tasks, and use of mobility aids like Hoyer lifts. Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times. Background in long-term care settings or hospice environments is advantageous. Proven ability to work effectively within multidisciplinary teams focused on patient-centered care.
