Your Mission:
Provide Exceptional, Holistic Care You will be the heart and hands of our operation, providing essential personal care and keen observation that directly impacts our patients' quality of life.
Direct Patient Care & Clinical Support:
Provide compassionate assistance with Activities of Daily Living (ADLs) : bathing, dressing, grooming, toileting, and safe mobility, using equipment like Hoyer lifts when necessary. Monitor and document patient observations , including vital signs (using blood pressure cuffs and pulse oximeters) and changes in condition, ensuring accurate records in our EHR system . Provide specialized, patient-centered care for individuals with dementia, Alzheimer's disease , and other memory impairments, utilizing proven behavior management strategies. Perform catheter care , assist with simple wound care , and provide essential support for hospice patients to ensure comfort, safety, and dignity during end-of-life care. Support patients with medication reminders under the supervision of a nurse, ensuring proper handling and documentation.
Holistic Home Support:
Assist with meal preparation, feeding, light housekeeping, and laundry to maintain a clean, safe, and healthy living environment. Support patients' emotional and social well-being through compassionate communication, active engagement, and respectful companionship. Employ sharp observation skills to detect subtle changes in condition or behavior and report your findings promptly to the supervising nurse.
Who You Are:
The Ideal Caregiver A certified Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in Missouri and/or Illinois .
A Experienced Caregiver:
Prior experience in assisted living, nursing homes, or home health is highly preferred. Experience with hospice, dementia care, or developmental disabilities is a significant advantage.
A Skilled Observer:
Knowledge of medical terminology , vital signs measurement, infection control protocols, and the ability to document accurately in EHR/EMR systems .
A Compassionate Professional:
You provide care with immense patience, empathy, and unwavering professionalism. CPR certification is required.
A Physical and Reliable Partner:
You have the physical stamina to safely lift or move patients using assistive devices and can manage long shifts. You are a reliable driver with a valid driver's license and auto insurance.
