Job Description

$17.85 per hour +

Alton, Illinois Job Duties:
Provide person-centered support to help clients achieve individual goals and maximize independence. Implement service plans, accommodations, and behavioral supports as assigned. Monitor client progress and communicate updates to the Employment Facilitator and team members. Maintain client dignity, confidentiality, and safety at all times. Assist clients with personal care, hygiene, meals, mobility, and transportation needs. Support clients during community outings and recreational activities. Serve as a bus monitor and provide transportation support as assigned. Train and support clients in vocational, life, social, and community skills. Assist clients with work opportunities, training activities, volunteer experiences, and community integration. Monitor performance and provide coaching, guidance, and encouragement. Prepare and maintain training areas, workspaces, and vehicles. Set up materials for activities and complete general cleaning and sanitation duties. Complete vehicle inspections and transportation documentation as required. Maintain accurate records of client services, progress, and program participation. Complete incident reports and required documentation in a timely manner. Utilize agency software systems and maintain confidentiality of client information. Requirements High School Diploma or G.E.D. Experience with people with developmental disabilities (DD) or mental illness (MI) is preferred. Basic computer operating / data entry skills required. Must pass a criminal background check. Must pass various State and Federal registry checks. CPR, 1st Aid, Crisis Prevention Institute (CPI) and DSP training provided by the company must successfully be completed within the first 4 months and annually thereafter to be certified and maintain position. Must pass DCFS Abuse and Neglect Tracking System check. Be 21 or older, have a valid driver's license, and pass a driving history check.
